Guidelines
APPLICANT ELIGIBILITY
Only eligible Summit Health, CityMD and VillageMD team members may receive assistance through the Team Fund, based on set criteria for catastrophic events causing the team member an unforeseen financial hardship. To be eligible for assistance from the Fund, the applicant must meet the following requirements:
● Be employed directly by Summit Health, CityMD, VillageMD or Starling Physicians for at least 1 year as a regular full -time team member.
● The event that resulted in a financial burden must be directly impactful to the applicant.
● Apply within 3 months after the event.
● Submit all supporting documentation in order to be considered for a Team Fund grant.
● Submit details related to the individual grant need, as requested (e.g., rent/mortgage, expenses, household income, etc.)
SITUATIONAL ELIGIBILITY
If the applicant has incurred extreme costs and his/her household income is below 400% of the federal poverty level, any of the following situations meet the Team Fund eligibility guidelines:
● Applicant or spouse/domestic partner has missed paid work time due to illness (for self, spouse/domestic partner or dependent) and is not receiving pay for such time off through any means.
● Applicant or dependent requires medically necessary treatment with long distance travel.
● Cost of medical treatment which causes material financial harm to an applicant or his/herdependent(s).
● Unforeseen disaster such as fire, flood, tornado or hurricane, which causes significant damage or destroys a home or apartment building.
● Homelessness due to domestic abuse.
● Burial or travel assistance is needed due to a death in the immediate family (e.g., team member’s parent or legal guardian, stepparent, brother, sister, current spouse/domestic partner, children, stepchildren, grandparents, grandchildren, current mother-in-law or current father-in-law).
● Other special situations, which prohibit the team member from maintaining his/her basic living needs.
When reviewing your application, your number of eligible dependents will be considered. Eligible dependents may include:
● Spouse (unless legally separated) or domestic partner
● Dependent children under age 19
● Dependent children under age 25 if reliant on applicant for support and currently a full-time student (as defined by the educational institution)
● Parent, current stepparent, current mother-in-law, current father-in-law
● Grandparent or grandchild
The Team Fund does not provide assistance for the following:
● When the situation is the result of poor financial management
● While the applicant is receiving Workers’ Compensation benefits
● When the situation is related to standard maternity/paternity leave and/or the situation results in payment of wages and/or health costs through another means
● Costs related to divorce or separation, including costs related to child support or non-receipt of child support
● Hardship resulting from taxes, job loss or loss of overtime (unless this is as a result of sudden disability or a death in the immediate family)
● When the situation does not threaten the team member’s ability to meet basic living expenses(e.g., While car problems are unfortunate and inconvenient, they do not threaten the ability to meet basic living expenses.)
● Past due bills not related to the unforeseen emergency (such as rent, utilities, car payments)
● Items covered by insurance or governmental assistance
● Elective, routine or ongoing medical expenses
● Routine school fees
● Illegal behavior or situations that occur while team member is under the influence of alcohol or controlled substances
● Event that is the result of an action taken by the team member (such as traffic violations, eviction due to repeated failure to pay rent, etc.)
● Personal bankruptcy
